Unless there was a more specific message (now that it's approved, I don't know), the generalist "did not approve" message just means that it ended up in their working queues but they did not get to it or make a decision on it.

I don't recall the exact wording, but it was something along the lines of '2 did not think it was of a high enough quality to approve', which sounds like they did make a decision on it. I could be mistaken though I guess, I don't know.
It's likely just the default fallback message from the system rather than mods actively mark it to be not of high quality. If it's high quality enough (like, rating 5 of 5) it will get instantly approved, but those that doesn't meet the cut is assumed to be "quality not high enough". That doesn't mean the quality is abysmal either, only not in that instant-approval level.
"No interest" is the default thing to click to hide the image (i.e. not approve it and get it out of your modqueue list) when the image isn't specifically "wrong" in some way. Those numbers don't go up automatically in any way as far as I know.
