Actually, the real truth is that Poison was in fact intended to be female in the original Final Fight, but they had some political issues going on in Japan at the time about violence against women. Because they were pretty much finished making the game, they didn't want to bother making an entirely new sprite for her and turned her into a tranny, or perhaps, judging by the obviously female body {no surgery can make a man look that feminine} and Ono's recent comments {he never outright said Poison was a man—he referred to her using exclusively female pronouns despite saying she has a penis}, a hermaphrodite. Capcom has only made the gender issue even more confusing and unresolved over the years, even going as far as including Poison in an official figurine set known as Capcom Girls Collection. I have to wonder why Capcom doesn't just retcon Poison into having always been female like Nintendo recently did with Birdo, since they seem more than happy to retcon anything else imaginable {such as how Charlie died, Sagat's entire backstory, and whether Gouken was killed or just sleeping}.
